Band 5 Community Nurse

Job Description

Position Overview:

We are looking for enthusiastic and skilled Band 5 Community Nurses who are passionate about delivering patient-centered care. As a Community Nurse, you will play a vital role in providing clinical nursing care to patients in a community setting, promoting independence, and improving their quality of life.

Key Responsibilities:

– Assess the needs of patients in their homes and develop individualized care plans.

– Administer medications, wound care, and other nursing interventions as prescribed.

– Monitor patients’ vital signs and assess their overall health status.

– Educate patients and their families on health management, medication administration, and self-care.

– Collaborate with a multidisciplinary healthcare team, including doctors, social workers, and therapists, to coordinate patient care.

– Provide emotional support and counseling to patients and their families.

– Document patient assessments, care plans, and interventions accurately and in a timely manner.

– Ensure compliance with infection control and safety protocols.

– Participate in on-call duties and emergency response as required.

Qualifications:

– Registered Nurse with current registration with the Nursing and Midwifery Council (NMC) in the UK.

– Previous nursing experience, preferably in a community or home healthcare setting.

– Strong clinical assessment and decision-making skills.

– Excellent communication and interpersonal abilities.

– Ability to work independently and as part of a collaborative team.

– Empathy, patience, and a genuine commitment to patient-centered care.

– Access to a reliable vehicle and a valid driver’s license (if required for the role).
